Transcript Information:
All applicants must include a high school transcript of grades from 9th grade and the first half of your 10th grade year. Have this section completed by the appropriate school official. Online transcripts and grade reports are not acceptable. You must also include the remainder of your sophomore year schedule. If this requirement is NOT complete, you will not be considered for the scholarship.
Deadline is January 31, 2009. PLEASE DOWNLOAD TRANSCRIPT FORM Or call 1-801-649-2286 to have one sent to you.
It is your responsibility to check with the school and make sure that they sent your transcripts before the deadline. No Late Transcripts Will Be Accepted. This can be sent in by the school or by the student.
Applicant Recommendations:
Two recommendations are required and must be completed in the format provided. If incomplete, your application will not be evaluated. The section is to be completed by a teacher, counselor advisor, or an instructor who knows you well. Preference will be given to references from math, science, and engineering teachers. Either DOWNLOAD THIS FORM and give it to the two people you will ask to make a recommendation and send in separately from the application, or have them fill it out online at the following url http://www.energysolutionsfoundation.org/scholar_recomend.php

APPLICATION FORM
The online application has three separate pages. You will be able to navigate back and forth on these pages. On the final submit your information will be sent to the Foundation. You will automatically receive a copy of your application via email if transmission was successful.
To be considered complete, the following material must be received by the EnergySolutions Foundation:
- __ Application filled out by 10th grade student.
- __ Current Complete Transcript(s) of Grades (Online transcripts are not acceptable.)
- __ Complete Copy of 10th Grade Schedule
- __ Essay. No handwritten essays accepted. (Topics listed in application)
- __ Two Recommendations sent in sealed envelope or by recommender.
- __ You have indicated an approved major of study.

Special notes for completing the application on the computer:
 Applicants must have Adobe Acrobat Reader to complete the application on the computer. If necessary, download the most recent version .- Applicants must complete the application at one time: data will not be saved.
- The application must be printed before hitting submit. Remember data does not get saved. (When complete and while the document is open, click on the File menu, choose Print, and click OK, then press submit.)
- After submitting, you will receive an email with a copy of your application as confirmation that is was submitted successfully. If you do not receive this email, we did not receive your application. You can either mail in a copy of your printed application or resubmit online.
- If you send in the essay separate from the rest of the application, it’s imperative that you include your state, school, name, and title of the essay. If your essay is not identified properly, your essay will not be matched up with your application and therefore your application will be considered incomplete.
Important Reminder!
Before leaving the application site, print the application for your records!
Data will not be saved.
The student is responsible for following up with the EnergySolutions Foundation to ensure that the Foundation received all materials.
